About Us

Dr. Daniel H. Kellum, Sr., D.O., F.A.A.F.P received his Medical degree from Kansas City University of Medicine and Biosciences in 1973.  He also has a Bachelor of Science degree in Pharmacy from University, Austin, Texas.  Dr. Kellum worked as a pharmacist for 10 years before becoming a physician.

He did his post graduate training at the Tulsa Regional Hospital in Tulsa, Oklahoma.  He is a fellow of the American Academy of Family Physicians, 1990 and Board Certification of Family Medicine 1993. 

He is active member of the Bexar County Medical Society, Texas Medical Association, Texas Osteopathic Medical Association, and The American Medical Association.

His purpose is to provide quality care to all his patients and strongly believes in promoting health and wellness.  He enjoys spending time with his lovely wife, family and friends.  He also likes movies, sports, the outdoors and playing baseball most weekends with the Men’s Senior Baseball League.

Denise Kellum, R.N., PA-C has been a family practice physician assistant since 2008.  A native San Antonian, she received her Bachelor of Science in Health Care Sciences and Master of Physician Assistant Studies from the University of Texas Health Science Center at San Antonio.  PA Kellum is board certified by the National Commission on Certification of Physician Assistants.  She is also an active member of the American Academy of Physician Assistants, Texas Academy of Physician Assistants, and Bexar County Physician Assistant Society.  PA Kellum values educating her patients in various stages of disease prevention.  Her hobbies include spending time and attending sporting events with her husband, playing with her dogs, and writing.

Mr. Matthew Arismendez was born and raised in San Antonio, Texas. He received his Bachelor's in Biology from St. Mary's University. Mr. Arismendez worked for four years as a sonographer performing, ultrasounds at a local hospital. Matt was inspired by several Physician Assistants he met at the hospital where he was working as sonographer.

Mr. Arismendez went back to school to receive his master's in physician Assistance studies in Albuquerque, New Mexico. He then returned to San Antonio, as there is no place like home. He enjoys being a Physician Assistant building relationships with his patients and empowering them to improve their health.

On his spare time, he enjoys spending time with his wife and five children and serving time at their church. He also enjoys songwriting and exercising as hobbies.

Alice Blackmon APRN, MSN, CPNP has been in the nursing field for over 29 years.  She received her Bachelors of Science degree in nursing in 1990 from University of Texas Health Science Center in San Antonio and her Masters of Nursing/Pediatric Nurse Practitioner degree in 1997 From Texas Women’s University in Houston.  Her experience ranges from hematology/oncology to pediatric HIV research to general pediatrics. 

She was a part of the Kellum Medical Group Family from 2000 to 2012 and has rejoined the practice as of April 2019. 

On her spare time she enjoys spending time with her three beautiful daughters and one grandson. 

Yolanda L Valdes earned a Masters Degree in Physician Assistant Studies (MPAS PA-C) and have been working for eight continuous years as a Physician Assistant with experience in both Internal and Family Medicine.

Prior to becoming a PA, I dedicated 10 years working in Research for a Medical Pathology laboratory here at the University of Texas Health Science Center in San Antonio, Texas.

Originally, I am from Mexico City, where I graduated with a Bachelors In Science with a Degree in Biology from the National University of Mexico City. I was awarded an Honorable Mention during my Professional Exam and on the basis of my final thesis.

I am bilingual and fluent in both Spanish and English, which have helped me to understand and provide medical care to patients with diverse cultural backgrounds.

I am happily married for more than 20 years and have two busy kids, my daughter and son. I love nature, animals, traveling and spending time with my family and friends, and of course my rescue greyhound, Coco!

Dr. Salvador C Vasquez M.D is Born and raised in San Antonio, Texas. He received his Bachelor's in Biology from St. Mary's University, with Honors Magna Cum Laude. He received his Medical degree from The University of Texas Health science Center at San Antonio, in 1988.

Dr. Vasquez MD gained tremendous experience by working as physician at Barrio Comprehensive Clinic and at University Health System SE Clinic. He takes holistic care of his patient.

Dr Salvador Vasquez MD loves history and sports and cars. He enjoys spending time reading about history and sports articles .

A proud member of Sigma Theta Tau International Honor Society of Nursing, Manuela received her Master of Science in Nursing as a Family Nurse Practitioner from the University of Texas Health Science Center at San Antonio in 2018!

Prior to joining the practice, she worked as a Family Nurse Practitioner in San Antonio, treating patients of all ages.

She loves being able to provide for patients and customize their care according to their needs and priorities.

Mrs. Berry enjoys spending her free time with her husband and three sons, relaxing by the pool, and traveling.
